Mi się wydaje że ta dioda która wskazał elmaya jest po stronie tranzystora, ja bym wymienił rezystor o oznaczeniu 471 czyli 470ohm zmień go na 220Ohm
W elektronice jak nie wiadomo o co chodzi to zwykle chodzi o zasilanie

they are interconnected just right on the PCB, if I understand You well. Besides that I'm looking on the pictures and something seems to be different on Your diagram but ... what you point out is the ground connection "DC-" for the output of the optocouplers.
check the connection between GND1, GND2, GND3, GND4 and DC- with the multimeter.

You were right. I've made a mistake (it was late at night) and my module has a LED in different place (picture). Something that in my mind was Your mistake was caused that the LED is after the optocoupler (not before) but it doesn't change anything. Bridge on the led solved problem, the dimmer is working but ... there's no connection between GND on input that You were writing about. What is it necessary for?
